When requesting information about our activities and services for example via the contact form on the website, e-mail, phone, text or other written communications.  
When you use the acuity link to book an appointment.
When  you fill out a medical history form.
When you communicate orally in your session.

 

What information is collected

Identification information such as your first name, last name, addresses (professional and/or personal), email addresses, date of birth, and phone numbers. 

Biographical information such as your job title, employer’s name, hobbies or sports, professional and/or personal background.

Medical history, medical reports you chose to share with us, as well as photos of you if we chose to take them as a tool to track progress.   We also keep service-related information in the form of clinical notes about the services we provided to you. 

We collect this information to communicate with you, provide you with accurate insurance receipts, and to best serve you.

 

Minors (clients under 14 years of age)

We do not knowingly collect information directly from children under the age of 14.  All children under the age of 14 must be accompanied to their session  by a parent or guardian and all information is collected with the consent of the parent/guardian..  

 

Use of Personal Information

We may use the personal information you provide about yourself to respond to your requests and questions related to our activities and services. By agreeing to provide us with personal information, you consent to its use for the purposes stated in this policy.

The personal information you provide to us is used to ensure that we can process your requests (bookings, comments, communications, etc.) that you are entitled to receive as a client.

Your personal information is only accessible to your osteopath.

We may disclose a user’s personal information for reasons expressly stipulated in the Private Sector Privacy Act C.P-39.1,  including to our lawyers, following the issuance of a subpoena or any other court order, or to prevent an act of violence.

We will confirm only the date, duration of appointment, price and name linked with an appointment to your insurance provider upon their request.

We take necessary measures to protect your collected personal information from any unauthorized disclosure. We will not use or transfer the personal information provided to us to another professional without your prior consent.

Your information is stored using the following programs:

Acuity Scheduling
Collects: Scheduling information, name, address, phone number and email.
Site specific cookies
This program is password protected and all devices where it is used are password protected.

Collects: Name, address, phone number, email, appointment date and time, duration and cost, invoice numbers,  type of payment and payments received.

Notability
Collects: Name, address, phone, email, medical history, clinical notes.
This is kept on an iPad which is password protected and these notes are kept in a dedicated folder which is also password protected.

 

Gmail and Google calendar
Collects: Your name, email address and any information you share with us in these messages.
When you email us either via the contact form or directly to the email address, you are connecting via gmail. 
Gmail is accessed on devices such as an iPhone, iPad or MacBook.  These are password protected and the mail server is password protected.

How long we keep your information:

We are required to keep clinical notes for 5 years.  After which time inactive files will be physically destroyed and digitally destroyed on all programs and devices.

 

Requesting a copy of your personal information

You may request a copy of your file at any time.  To do so please connect with us by email.
We will respond to your request by email within 30 days of your request.
The file will be ready within 45 days of the request and will have to be picked up in person.  The client will be asked to provide ID at the time of pick up.

 

Reporting a concern regarding privacy 

Your privacy is of utmost importance to us.  Please contact us by email with any concerns or questions.  We will respond by email within 30 days of any concern or complaint.
